Work related breaches and wireless breaches are common and sometimes require the services of an IT Security Professional. I've never experienced any myself but if your looking for information here are a couple of links. The first are case files from security monkey they are a very good read. http://it.toolbox.com/blogs/securitymonkey/official-securitymonkey-case-file-index-14787

The next is about security in general from the Information Technology
Professional Community.

Is it considered a security breach of you leave the front door open with a big welcome sign hanging over it?

I set up a publicly available anonymous ftp server on my home computer once; somebody found it and started using it to store/transfer their own stuff. In doing so, they created a bunch of folders with weird filenames that were very difficult to delete. I forget exactly how I got rid of them, but it took a while.
